At Closer Care, we believe getting a little extra support shouldn’t mean giving up your independence.
We’re expanding our services to support older Australians who want to continue living safely, confidently and independently in their own homes and communities.
Through our partnership with Trilogy Care, Closer Care can provide personalised aged care supports under the Support at Home program, delivered with the same relationship focused approach that has always been at the heart of what we do.
Support that feels personal
We know inviting someone into your home is a big decision.
That’s why we place such a strong focus on finding the right people, building genuine relationships and understanding what matters to you.
Whether you need some help around the home, support getting out into the community, assistance with everyday activities or simply some regular companionship, our goal is to make life a little easier while helping you maintain as much independence as possible.
How we can support you
Depending on your individual needs, funding and care plan, supports may include:
- 𝐒𝐨𝐜𝐢𝐚𝐥 𝐬𝐮𝐩𝐩𝐨𝐫𝐭 𝐚𝐧𝐝 𝐜𝐨𝐦𝐩𝐚𝐧𝐢𝐨𝐧𝐬𝐡𝐢𝐩
Someone to spend time with, have a chat, enjoy activities with or simply provide some company.

Support to attend appointments, go shopping, visit friends and family, enjoy a coffee, attend activities or stay connected with your local community.
- 𝐇𝐞𝐥𝐩 𝐚𝐫𝐨𝐮𝐧𝐝 𝐭𝐡𝐞 𝐡𝐨𝐦𝐞
Practical assistance with everyday household tasks to help you continue living comfortably and safely at home.

Respectful assistance with everyday routines and personal care where appropriate.

Support getting to medical appointments, community activities, shopping and other important places.

It starts with getting to know you
There is no one size fits all approach to care.
Some people may need regular support throughout the week. Others might only need a few hours of assistance, some companionship or help getting out and about.
We want to understand who you are, what is important to you and what you would like support with.
From there, we can help you understand what the next steps may look like and whether Closer Care, through our partnership with Trilogy Care, may be able to support you.
Support for families too
Finding support for a parent, grandparent, partner or loved one can feel overwhelming, particularly if you’re navigating aged care for the first time.
You don’t have to understand everything before contacting us.
If you’re trying to work out what support your loved one may need, how Support at Home works or where to begin, you’re welcome to have a conversation with our team.
Sometimes the first step is simply having someone to talk it through with.
