Support at Home in Sydney: A New Chapter for Closer Care

Home is where we feel comfortable. It’s where we know our neighbours, follow our routines and keep the things that matter most close by. As we get older, a little extra help can make it easier to keep enjoying that familiar life.

We’re pleased to share that Closer Care now offers aged care support through our partnership with Trilogy Care, a registered aged care provider. This new chapter brings our personal, relationship-focused approach to older people and families looking for support at home in Sydney.

What is Support at Home?

Support at Home is an Australian Government aged care program that helps eligible older people continue living independently at home. It replaced the Home Care Packages Program and the Short-Term Restorative Care Programme in November 2025.

The support available depends on your assessed needs, approved services and funding. It can include help with everyday tasks, personal care and staying connected with your community.

How can Closer Care help?

Through our partnership with Trilogy Care, we can discuss support that suits your needs, care plan and funding, including:

  • Help around the home: assistance with everyday household tasks.

  • Personal care: respectful support with daily routines.

  • Social support and companionship: someone to talk with and enjoy activities alongside.

  • Community access: help to continue seeing people and doing things you enjoy.

  • Transport and appointments: assistance getting to shopping, appointments and community activities.

Perhaps you’re finding household tasks more tiring, or you’d like some company when heading out. You might be a family member exploring options for a parent. Whatever brings you here, the conversation starts with what would make everyday life easier.

Care starts with getting to know you

Inviting someone into your home is personal. You deserve to feel heard, respected and comfortable with the support you receive.

At Closer Care, we take time to understand your preferences, routines and goals. Our approach puts relationships at the centre of care, with support shaped around what matters to you.

That might mean having help to maintain a comfortable home, keeping a regular outing on the calendar or feeling more confident attending appointments.

Where do you begin?

If you’re new to government-funded aged care, My Aged Care is the starting point. An aged care assessment determines your eligibility and the services you may receive. Visit My Aged Care or call 1800 200 422 to get started.

When services can begin depends on approval, funding availability and arrangements with your provider. You may need to contribute towards the cost of some services, depending on your circumstances and the support you receive.

If you already have Support at Home funding, our team can discuss your needs and whether Closer Care can provide suitable support through our partnership with Trilogy Care.

Let’s talk about support at home

You don’t need to have every answer before reaching out. Whether you’re looking for yourself or someone you love, we welcome a conversation about your needs and the next steps.

Call Closer Care on (02) 8359 1591 or visit our Support at Home page to enquire.
